By signing, you are agreeing to the following terms: 1. I understand that all medical care for our foster animals will be provided through Deerfield Animal Care Center (DACC), and Orphans of the Storm (Hereinafter “OOTS”) will NOT reimburse me for the cost of ANY medical care outside of DACC or as previously authorized 2. I will carefully read and follow all instructions provided by OOTS. 3. I am at least 18 years of age. 4. I agree to return my foster animal at any time upon the request of the OOTS Foster Coordinator or any OOTS employee. 5. I understand that I am responsible for the health of my own animals and their vaccine status. Orphans of the Storm is not responsible for any contagious disease that my animals may be exposed to due to fostering a shelter animal. 6. I will not introduce my foster animals to any other animals other than those that reside in my home. 7. I will not take my foster dog to a dog park or any other off-leash area or let my foster dog off leash. 8. I will not leave my foster dog unattended in a fenced yard. 9. I will immediately notify the Foster Coordinator if my foster animal gets loose or is lost. 10. I will not travel out of the area with my foster animal. 11. I will not hand off my foster animal to a sitter or another foster without prior approval from the Foster Coordinator. 12. I confirm that I am willingly offering to foster and that I do not expect any goods or services in return, including an adoption fee reduction or waiver should I decide to adopt my foster animal. 13. I will actively participate in marketing my foster animal and be responsive and courteous towards each potential adopter pursuant to the adoption process protocol provided to me. 14. I acknowledge that the decision as to who will adopt my foster animal rests with OOTS. 15. I will not hand off my foster animal to a potential adopter or let them take my foster animal on a trial basis, field trip, or sleepover before the completion of the official adoption process. 16. I acknowledge the risks involved in fostering animals, including bites, scratches, injury, death and property damage. I understand that while OOTS staf carefully monitors all animals, OOTS may have limited knowledge of their behavioral and medical backgrounds. Therefore, I understand that OOTS makes no warranties or representations regarding my foster animal’s health, behavior, temperament, age, or breed. I further understand that environmental changes may affect and change the temperament of my foster animal(s).I swear that I am participating voluntarily and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity. 17. I will immediately report any foster animal bite pursuant to the Foster Bite Protocol provided to me. 18. I will make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed. 19. I understand and agree that Orphans of the Storm, its members, officers, directors, agents and elected officials have no liability or responsibility of any nature for injuries or damage to any person, animal, or property which may be caused by my foster animal(s) or my participation in the OOTS Foster Program. I, for myself and my heirs, executors, administrators and assigns, hereby release, indemnify and hold harmless OOTS, its employees, officers, directors, agents, and their heirs, administrators, executors, successors, and representatives from all liability for and all risk of damage or bodily injury or death that may occur to me (including any injury based on Orphans of the Storm’s negligence) now or hereafter in connection with my foster animal(s) and my participation in the OOTS Foster Program. I expressly agree that this release, waiver and indemnity is intended to be as broad and inclusive as permitted by the State of Illinois and that if any portion is held invalid, the remainder shall continue in full force and effect.
